The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to fill out our New Patient Questionaire prior to your appointment, allows us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental Insurance

As a courtesy to our patients, we will file most insurance claims.  We are committed to helping you maximize your insurance benefits however, insurance policies vary & we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service.  As a courtesy to our patients, we attempt to help you receive full insurance benefits; although, you are personally responsible for your account, & we encourage you to contact us if your insurance has not paid within 30 days.  Our experienced office staff is well-versed in dealing with insurance and we will be happy to assist you.

Laramie Dental Arts, P.C. are preferred providers for Delta Dental and Blue Cross/Blue Shield.

Financing

CareCredit lets you begin your treatment immediately — then pay for it over time with low monthly payments that fit easily into your monthly budget.  One of the greatest services is CareCredit's no interest payment plans.
